1. Acknowledgement of Spray Tan Procedure
I understand that spray tanning involves the application of a cosmetic solution to the skin to achieve a temporary tanned appearance. The results can vary depending on my skin type, preparation, and aftercare.
I acknowledge that spray tanning is generally safe, but as with any cosmetic product, there is a risk of allergic reactions or skin irritation.
I understand that the solution does not contain sunscreen and will not protect against sunburn.
I release Glow by Sydney Hope from responsibility for reactions that may occur due to undisclosed medical conditions, medications, or failure to follow pre- and post-care instructions.
2. Client Responsibilities
Arrive with clean, exfoliated skin free of lotion, oils, deodorant, or makeup.
Follow aftercare instructions provided to prolong the tan.
Understand that results may vary and longevity depends on my skincare routine and lifestyle.
3. Staining and Responsibility
I understand that DHA (the active ingredient in tanning solution) may temporarily stain clothing, bedding, or other materials. I release Glow by Sydney Hope from liability for any staining of personal items.
4. Health Conditions
I affirm that I do not have respiratory conditions, open wounds, skin infections, or contagious conditions that may be affected by spray tanning.
If pregnant or breastfeeding, I have consulted with my healthcare provider and accept full responsibility for proceeding with the service.
5. Consent and Liability Release
I consent to receive spray tanning services at Glow by Sydney Hope. I understand the risks and have disclosed all relevant medical information. I release Glow by Sydney Hope, its staff, and affiliates from any liability related to my spray tanning session.
